By filling out this form, you agree to pay the invoice of $1,739.51 plus taxes, which will be sent to your insurance company at the end of the mandate. If your insurance company does not cover or pay this invoice, or if you do not have insurance, you will be responsible for the full payment. You may submit the invoice to your insurance company for reimbursement, if applicable. In case of non-payment, we reserve the right to take legal action to recover the amount owed.
